This orientation and training guide provides information about the philosophy and goals of hospice and outlines the responsibilities of hospices and nursing facility staffs to care for facility residents as they near the end of life. The training guide may be downloaded at any time by facility leadership to orient and train their staff about hospice care. (54 pages)

Orientation Completion Test
The Orientation Completion Test assesses nursing facility staff competency and understanding about hospice care provided in the nursing facility. (2 pages)

Certificate of Completion
The Certificate of Completion should be completed by facility leadership following the successful completion of the Orientation Completion Test.  A copy of the Certificate of Completion should be placed in the facility employee’s file for survey compliance.  A copy should also be provided to the facility employee. (1 page)

Contract Addendum Model
The contract between the hospice and the nursing facility should be amended to include language about facility staff training and documentation of training to meet regulatory requirements(1 page)
