Accreditation and Compliance

The Oregon Hospice Association’s commitment to ensuring and promoting access to high quality hospice and comfort care for all is demonstrated through its Accreditation and Technical Assistance program.

OHA accreditation satisfies the requirement for accreditation under ORS 443.860(1)(b) (January 1, 2008). Intended to support hospice providers improve standards of care and meet regulatory requirements on a continuing basis, OHA accreditation surveys are staff and/or peer reviews conducted by two surveyors (OHA’s technical assistance advisor(s) and one hospice RN) over a 2-3 day time period.

Survey Process

The survey process relies on the State Operations Manual – Appendix M – Guidelines for Surveyors and NHPCO’s Standards of Practice for Hospice Programs, which provides parameters for “acceptable standards of practice”. Regulatory aspects of the survey include compliance with all applicable state laws. Also integral to the process is an analysis of comparative data. Attendance at team meetings and home visits provide further opportunities for in-depth assessments of quality of care. The survey concludes with an exit interview. A written summary of findings and recommendations is forwarded to programs within a week of survey completion.

Upon analysis of the findings, the surveyors recommend to Oregon Hospice Association's Board of Directors that (1) a hospice be granted a) OHA accreditation status or b) OHA accreditation status “Pending”; or (2) a hospice be denied accreditation.

"Pending" status is recommended when the surveyors are of the opinion the agency does not satisfy certain regulatory requirements or acceptable standards of practice. The agency has an opportunity to submit to OHA plans of correction to address the issues identified by the surveyors. A follow up visit by surveyors may be necessary.

If surveyors are of the opinion that an agency does not meet regulatory requirements or offers substandard levels of care, accreditation will not be recommended.

The State Health Division will be kept informed of changes in a hospice program's OHA accreditation status. The State Health Division will be notified immediately, if the surveyors' findings reveal practices that endanger patient/family well-being.

Fees

OHA accreditation surveys cost $3,000 (plus expenses) for first-time applicants. Programs seeking re-accreditation status are charged $2,000 (plus expenses). OHA accreditation status is valid for 2 years.
